Filing a Claim:

To start filing a claim, please enter your certificate or sales invoice number and zip code. Or enter last and first name and zip code. After entering the information click on the "Get Certificate" button.

Important: Use only letters (A-Z), numbers (0-9), remove any period (.), hyphen (-), pound (#), or spaces from your sales invoice or certificate number. No other characters are allowed.
If you have more than one certificate number, enter one at a time or the one which you want to file a claim in.

Important Note: To start filing a claim, you must enter the zip code that exists in our databases. If you have moved or your zip code has changed, enter the zip code contained in your service contract. If your zip code has changed and you sent us an address change notification, you must use your new zip code. Also your first and last name must match your printed agreement.
